Jessamine Humane Society Adoption Contract

agreement with the Jessamine Humane Society (JHS, business address: PO Box 101, Nicholasville, KY 40340-0101)
and do hereby acknowledge receipt of the following animal:

I understand I must comply, accept and agree to fulfill the following conditions and requirements:
1. If a veterinarian certifies in writing to the JHS that the animal is NOT in an adoptable condition
because of severe illness or abnormality, I will receive a refund of the adoption fee or another animal,
if available, upon returning the adopted animal within a period of ten (10) days after adoption.
Refunds will be mailed within thirty (30) days of receipt of the animal and necessary documentation.
2. Refunds for any reason other than those stated above, will not be given.
3. The animal shall be provided with proper food, clean water, appropriate shelter, proper veterinarian care,
and kind treatment at all times.
4. If at any time, I can no longer care for, keep or want this animal,
I will return it to the Jessamine Humane Society. I will not be eligible for a refund of fees paid.
5. I will follow through with the necessary medical vaccinations as they become due.
I understand the cat adoption fee of $75.00 or the dog adoption fee of $80.00 includes spay or neuter,
initial deworming, FeLUK-FIV test (cats), the first in a series of three shots - DHLPP (dogs) or FVRCCP (cats),
AVID micro-chipping, and rabies vaccination. Additional vaccinations will be the financial responsibility
of the new owner.
I have read the above and accept the terms of this contract. I am above the age of twenty-one (21) years.
